Well, phone screens tend to be quite a bit smaller, the smaller elements on screen may cause more eye strain. It could also be that when you hold your phone you have to look down at it to read. This is an ergonomic compromise that very frequently causes neck/shoulder/head pain. Very common.

Could also be where you read. Do you have back support? Are you lying down with a pillow propped behind your head? Are you standing? Sitting on the bus/subway? Etc. All of these scenarios have ergonomic pitfalls that accompany them. And it is only a myth that you have to do something in the wrong ergonomic position for HOURS to feel any pain. You may feel it rather quickly, it all just depends.
