You have to use an Amazon Rewards Visa card as your payment method for $100 or more in merchandise, then enter the code PRI20ARC. It doesn't work for everything. If you have more than one item, you have to have the same shipping speed checked for everything.
The $5 no-rush credit shows up at checkout if available--it's instead of the usual $1 credit for digital content. And the credit is only applicable to certain departments.
