For the life of me I cannot remember what I did last time to fix this, so... Sorry!

Once I add an image as a ebook cover, save, then click individually convert, then drag&drop the files to my Kindle; the ebook will work fine on the device, however the cover will be the default blank Amazon cover.

How do I fix this?
