What happens when you try to open the ToC on the Glo HD? Is it empty or do you get an error?

How big is the file and how big is the ToC? There was a bug reported recently with a large book that had a lot of ToC entries. It appears there is an issue with this in the recent firmware. But, in this case, the book wouldn't open at all and there was an error displayed. That problem has been reported to Kobo and hopefully will be fixed in the next firmware.

For yours, it might be an error in the ToC. The calibre viewer is a lot more forgiving of errors in an epub than the Kobo firmware. So, IanD's suggestion of regenerating the ToC using Sigil or calibre is a good idea. After doing this, the ToC will definitely be correct. If you do this, make sure you delete the book from the device before resending it. This is to remove details of the book, including the ToC, from the internal database.
