Launching KOreader from the listing(s) of the books present is a separate KOreader add-in.

KOreader's view of the book organization on your Kindle is based on the directory structure, it does not use the "Collections" database on the Kindle.

We do have an add-in that will create/maintain the Kindle's collections database, based on the directory structure of the documents on the Kindle.

Here, the features you are interested in are not jumbled together in one (mostly stolen) glop of code.

Having a K4 may limit your choices of add-ins here, since its keyboard is different than either the physical or the touch keyboard devices.
I.E: Add-ins for the K4 that need user keyboard interaction must be specifically written for the K4.

Such as the KOreader / Liberator dual of add-ins.
