I had that problem with one day of titles, though fortunately it was one I didn't care about. I found the reason for it was that I just went to the title through the app and hit download. Since it was the free title for the day it worked but treated me like I didn't have an account. To make sure this didn't happen to me again I followed the instructions you suggested: instead of clicking download, I clicked on the streaming option (I do it from the website on my computer but this would likely work from the app as well). After the preview streams it asks if you want to continue. If you say yes the title gets added to your account.
While I've been happy with their service, I hate their web site and app. Just let me d/l to my computer and do everything from there. From that standpoint I think Downpour is much better.
