Yes, of course you were quite right. After reinstalling I got the same problem with my old koreader installation. It turned out that my customized sed-statements slowly killed the koreader.sh script. It turned the reader.lua line into one with a lot of repeating options. Each time I called koreader from KSM an option was added. Not harmful, but apparently also no executable after half a terminal screen of options.
In effect it felt a bit like a rather late spring cleaning of my ereader

EDIT: Rotation issue remains, so I'll accept your invitation, too. The script logs were generated on a Kobo Aura H2O.
