I gave up on the "use first image" settings, because I got tired of having the damned twitter bird as an cover page :P
There's a specific feature for excluding that sort of thing (cover_exclusion_regexp). I try to put common ones in the defaults.ini when I know about them. I don't see a twitter bird in this story.

It looks like storiesonline.net may have added a cover image feature--or we didn't know about it before. (I didn't write the code for this site and I don't read it.)

Can you provide other examples of stories with covers like that--preferably from more than one author? I need a bigger sample set to insure it gets implemented correctly.

the cover gets over-written or flattened
On the FanFicFare config Calibre Cover tab, you might change Update Calibre Cover (from EPUB) to 'Yes, if EPUB has image' instead of just 'Yes'.

I don't know what 'flattened' means in this context.
