Thank you Eschwartz, I did not know this.

There is an actual page with a reference to the missing image in that file. Kindle Previewer simply renders it as a blank page, in the calibre viewer you get the blue missing image icon. Open the azw3 file in the calibre editor and delete last html file and you will be fine.

I can understand that I (unintentionally) broke a forum rule. What I can't understand is the overzealousness of the moderator who banned me for a first time offence. A warning would have been sufficient and the offending item removed.
