From my own experience and what I've seen here and at Alf's:
I believe that all accounts have now been switched to non-credit card based keys. Most accounts have one current key which is always the same regardless of download method. A few accounts have a second active key which is only used on Nookstudy downloads. So far I haven't seen anyone report that their key has changed again after being switched to a non-cc based key.
Older downloads will have whatever key was current at the time they were downloaded. As far as I know, all Nook apps/devices will store keys for anything they have previously opened.
