After trying all the suggestions and failing to solve the issue, I remembered that I had Calibre 2.0 on an older machine with the DRM plug-in that worked.
So I removed Calibre's latest version and installed 2.0, then the plug-in, and it now works fine.

Somehow the latest version seems to have a conflict with something in my machine. With 10 TB of applications and data, it would have been far too much work to try to find the conflict (and still might not have). But 2.0 does everything I need from Calibre, so I'm now a happy camper.
