@Fbone - just the calibre config folder and the library folder(s).
Don't worry about calibre-cache its dynamic data that gets created as needed.
If
you created any file associations referencing calibre programs like the viewer and editor then you'll need to recreate after reinstalling windows and calibre. Otherwise there's nothing in registry except what's put there by the MS installer, when you do a fresh install then the MS installer will take care of that.
The Program Files... folder only has what's put there by the installer - no user data.
If you're reinstalling Windows why don't you do an inplace-reinstall, then you wont need to reinstall applications, restore data etc. An inplace-reinstall of Windows 7 will leave everything except Windows as is - including registry dependent software. You'd still want to backup data in case anything went pear shaped, but my experience is that it saves many hours of work.
