Well WOJ has repeatedly answered he's as dead as you can be. Personally IMHO, I belong to theory that it's future and/or alternate Dresden/Elaine.
In recent interviews, WOJ has said he has always planned a book currently titled "Mirror Mirror". In Which Harry see's how his life would of turned out if he had made a different choice when he was younger. Unless he's going to do A Dresden's "Christmas Carol", I think it's going to be more of a manifestation of Harry doing something stupid (perhaps w/ Elaine by his side so it affects her too). I'm theorizing that it creates nega-Dresden/Elaine or perhaps sends them through time, which we know he's going to break that law at some point. Where they both take every opportunity to accept more power, opposite our heroes. Evidence mostly from their second encounter in DB:
1. Harry describes Cowl as stronger than anyone he's faced including McCoy, DuMorne, Wardens, and least of the Queens. He goes on to say that he hasn't felt a power like this, although it is not inhuman. While it doesn't have the twisted energies of the Cowl's fellow necromancers or dark practitioners, "there's a lingering sense of black magic involved in his power." He goes on to say "Then again, there's a little in mine too."
+ He pretty much describe's Cowl just about how everyone else describes Harry. And he immediately calls Cowl's magic feeling very similar to his own and could be argued as a foreshadow.
2. In that fight, did anyone else notice that every time "Cowl" drew in power to attack there just happened to be a "cold like wind" that blew off the lake.
+ Similar to how maybe a Winter's Mantle might draw in power?
3. Cowl hasn't killed anyone to my knowledge. (At least that we're aware of)
+ He could of killed Harry, he didn't. Could of probably killed Carlos, he didn't. Should of ordered Kumori to kill Harry, he didn't and she didn't kill Harry even though it seemed she had the opportunity when he made his move to stop the dark hallow.
We do see him bring in an army of ghouls to kill the White Court (I won't lose any sleep over them.) May have been responsible for opening the portal to the Warden training camp but wasn't the one to kill the twin students. Although he was manipulating those events pretty much allowing the death of magic sensitive women, but again he never actually killed them.
Of course, I don't know whether they're an alternate version of them, or perhaps Harry and Elaine having a Prisoner of Azkaban moments. I do know in PG, someone in a silver car ran into Harry while he was driving and someone who knew him well enough to fix Little Chicago in which saved him (these events have yet be explained). Lea was a Sidhe-sickle and Mab was under siege at that time. Possibly Elaine or future Harry? Also in Changes, someone else was running around in the background yelling "fuego" in which Harry responds "that's my line" a red herring or another possible foreshadow of time traveling Harry?
