Don't concern yourself with the folder structure of a calibre library, it's largely irrelevant. The core (driving force) of calibre is its database, you can sort and select on its columns and you can add your own columns. For study notes you might want to use 'virtual libraries' for subject domains - eg pathology, surgery, psychiatry etc.
