originates from Windows - it gets passed up the stack to calibre via Python, its quite low level so one of the developers (kovid or chaley) will probably have to diagnose.
It could be caused by a 3rd party program. Try starting Windows in Safe Mode and running portable from there. If that works then restart Windows in Normal Mode and terminate ALL other programs - metro apps, regular windows apps, including those running 'in the notification zone' and try again. If it works then you'll have to find out which of the other programs is 'getting in the way' by a process of elimination.
BTW its better to post calibre messages as text (i.e. hit the copy to clipboard button and paste here with ctrl/v) then us responders can select/copy bits of text to use in searches etc. It's also easier for you.
