That could probably be done similar to the way it works on the Nook Simple Touch. The app creates a new folder in the screen saver area called "currentbook", and it contains a single image that was the cover of the last EPUB that the app viewed. Then in the NST screen configuration area, you can select "currentbook" as the screen saver and it's done. Any time you open a new EPUB, the screen saver is changed. Since the app supports file types without metadata and/or packaged covers, you could perhaps have it look for a file named "filename_COVER.jpg" or something when opening the file, and use that instead if there isn't an internal file to extract (and make that behavior default and supercede the attempt to extract one in case a user wants to supply their own for any book file).
