It occurs to me that the second of these issues may perhaps be the one that the OP is seeing.
As for 1, as long as you have all of your keys correct, the pre-QT5 version would still work. It was just the UI that bombed on Calibre 2.x.

I did encounter the issue with 2 on pre-2.x Calibre. I had no trouble with ePub in large batches. It was Kindle eBooks that had the problem. I added Kindle eBooks in no more than 5 at a time.
