Basically, you create the source HTML or EPUB the usual way, but with a bunch of weird rules about which CSS selector combinations are supported (a bunch of stuff just doesn't work). Then you run kindlegen, test it with Kindle Previewer, swear at how wrong the output is, rinse, and repeat.
