You can't have the same tag with different case. You get one or the other, which is why the choice is "don't change the case" so the "hidden" tags aren't changed or "change the case" including the "hidden" tags.

I have submitted code to Kovid that implements VL-aware tag changing/deleting in the tag browser. He said he will be able to look at it in several days.
