The format is really required because Android stores user information in some hidden place. The fact that you aren't seeing the the private folder supports the theory that Android is refusing to create it.

See here and here for some instructions on how to format an SD card in Android.

Beyond this I have no idea. What I can say with certainty is that Android is refusing to provide CC with the path to the SD card.
