It's not just Smashwords books that are not added to the library.
I've encountered the same problem with Dutch language books of Dutch publishers. On a few Dutch webforums others have mentioned the same problems.
Some even received a personal promo code that didn't work. The customer had prove she/he received a coupon code (sending screenshot of the mailing).
Information that must be readily available or retreivable at Kobos back office.
Kobo's answer on books that couldn't be downloaded:

After 4 days the book still couldn't be downloaded and also not after 10 days.
Note: Kobo has solved it by issueing a shop credit. It was not possible to add this book manually and/or Kobo could not replace/exchange it with another book title.
