The high rating and glowing reviews leads me to post the following book. I know that it will be of "appeal" only to a small number of people who follow this thread. Perhaps it will be of help to many of them.

One thing that reviewers frequently mention is that the book presents a lot of research results. Like probably all medical problems, there are rapid advances in the understanding and treatment of this one, and information on understanding and treatment of it quickly becomes out-of-date.
