You have to buy something first, and you have to buy it from a qualifying country, which the store set to the right language (I've been picking up francophone books from the fr-CA version of Kobo for most recent batches of purchases, so I didn't get to see the message for a while and thought they'd finished the offer, even though my main account which I've been using to buy with, is for Canada, though I did see it once in the UK when I bought a non-couponable sale book a couple of days ago). The widget loads on the confirmation page after your purchase (sometimes it might take a while for it to load).
IIRC, technically you've got 4 options when the widget comes up, Twitter, Facebook, some kind of app that I can't recall, and email. Click one of those options and it'll pop up some sort of text for you to approve sending which includes the 20% code and a generic shortened link for a list of suggested books. I haven't set up a new account via referral, since I prefer to keep all my actual paid purchases in just my 3 accounts for the regions I buy in and I'm willing to wait for the next coupon contest to come around, since I'm not shopping outside the usual countries.
It's always identical generic text with no tracking information as far as I can tell, and no apparent way for them to tell who you sent it to or if they used it or even who sent it to the hypothetical new account, so I've no idea if they can even follow through on the 50%, or if they'll just send a generic 50% to everyone who's technically clicked to do the promo, the way they sent a follow-up code to everyone who participated in one of their contests a while ago.
