I don't use the height instruction, just width="100%".

There are formatters who don't approve of this approach, but it seems to work pretty well for me, on an e-ink Kindle, Fire tablet, Nook reader, and various apps. Since I'm not a fan of Apple's closed universe, I don't own (and won't buy) an iPad. I have however looked at my books on a friend's iPad and on my iPhone 4, and the images look good there as well.
