There's a chance you didn't change it. It's been awhile since I've down a new install so I don't remember what the default setting is for that. It may default to being unchecked.

Which version of Calibre are you using? My computer at work is running v1.48, yay XP and cheap bosses , and that specifically says "Read metadata from file contents rather than file name". I can't see why Kovid would have changed that wording in later versions or in the Mac/Linux versions.

And, yeah, that would be interesting to see Calibre try to pull a cover image from a file name.
