Has anyone reported the bug to Kobo? I know I haven't. When I saw it and worked out what was happening, it was because I was cheating. I did something the device didn't support. Then I fixed it for my book, finished the book and forgot about it. Hence, I didn't report it. With what is happening with your book, I can reproduce it in a way that is just using the device. That is a bug I can report. Hopefully, I'll have time over the weekend to write it up properly.
Yes, it would be frustrating, and it would probably lead to a factory reset.

Re the Annotations bug, I hadn't realised that it hadn't been reported (or it's unknown if it's been reported), it seems to have been around quite a long time. (Maybe approx 1 year?)
David please don't do the bug report unless you really do have time (although if you're looking for excuses to get out of the Christmas shopping at overcrowded malls I can understand your reasoning
