I googled on the the kobo_extra.css file but as far as I understand this will only in place when sending the book to the device.
It will not change the books already on the device,and a patch would.
But the idea is interesting so I will Google some more about options for this .css for future sending.

Yes, it only affects books as they are sent to the device. The details are in the
announcement post for that feature. And there is some discussion following it. Have a read of that and see if it answers your questions.
The other possibility is if you are using kepubs. There is a user styles file that can override the settings in the books. It doesn't override everything, but it can help. And it completely escapes me what the name of the file is. A search should find it.
