I'm guessing you have Preferences->Add Books set to read metadata from the file contents, and I suspect the umlauts aren't in the embedded metadata. To prove my assertion open the book in the calibre viewer (press 'V' select EPUB) and then click the blue 'I' in the toolbar.

If the cover is in the EPUB then see screen shot to see how to extract it.
I put sub titles into a separate custom column.
