That's how someone talks to a baby:
"Ohhh, did you make a 'boo-boo'?" This means did you make a mistake. This is usually followed by loving gestures and words of encouragement.
In the corporate world, however, one would not say
"I just made a 'boo-boo' on those quarterly reports." This is likely to get one fired.
Talking that way to a baby, of course, is quite acceptable.
