I had completely forgot about this, but, Kobo removed the annotation option from the menu when reading PDF in 3.11.0. But, they didn't take away the ability to create a bookmark. There are two ways to get to them. From the tile and from the library list. If you long press on a book in the library, the menu will have an "Annotations" option if the book has annotations.

As to the percent into the book, that has been broken for a long time. The percent is recorded correctly, but if an epub or PDF is on the main memory, when it is reopened, the percent will be set to zero. But, sorting the annotations by location should work correctly. It does for epubs, but I haven't tested PDFs recently.
