That is definitely not the case. I frequently have both the purchased kepub and the DRM freed epub on my device. Plus at times I sideload as a kepub to test something. Or simply put the epub in a different spot so I can have two copies to compare. Or because I changed the file template and resent a book.

The key used in the database is based on the full file path of a sideloaded book. For purchased kepubs, it is an id from Kobo which is used as the file name.
