I ran into a very similar problem a few weeks ago. I discovered through some frustrating trial and error that, for some reason, the "key" for that book used just my last name, not my full name as it appears on the credit card. I generated a second key file and haven't had a problem since.

Try a few variations on your name and see if that helps.
