By default it is set to that, though. I assume anyone who manually changed it would know.
Also, in that case it wouldn't be doing anything regardless; the OP states that double-clicking tries to add it.

However if an association has been set for TPZ in OS/X that references calibre.exe then that would probably cause calibre to try to add the book again. But I very doubt calibre itself would have established such an association.

I don't know how that association got set (it is certainly set on my linux machines -- by calibre) but it is the only explanation I can think of, and it fits the problem.
