That is the problem. When you open the book for viewing, it is probably not configured to open with the internal viewer.
So calibre passes the file to the OS to open, and the OS is configured to open .tpz files with the calibre GUI.
When you open a file with the calibre GUI, it gets added to the library.
You must go to Preferences ===> Look & Feel ==> Behavior ==> Use internal viewer for:
and tick the box for TPZ.

Actually, no. There is no TPZ option, I think calibre doesn't understand it. I guess because the only ones in the wild are from Amazon and very possibly DRM-locked. And the plugin-that-must-not-be-named renames it to HTMLZ.
(You can capture the log file by running calibre from the command-line, using "calibre-debug -g", or by clicking the dropdown by Preferences and selecting Restart in debug mode.)
