I think you should provide .mobi because older Kindles are unable to read .azw3 format. And there are still millions of older Kindles in use.

What did you mean by text boxes? Traditionally, that would mean something like <input type="text" size="30" name="box1">, but I don't know how that could work, since it would require a form with action to a URL, and I don't think that would work well.
