The "real page numbers" are stored both as page targets in the .mobi file and in a separate .apnx file, which might have been generated from either pagelists or pagemaps in the original ePubs. (Example files are in this thread.)

AFAIK, Calibre cannot convert .mobi books with .apnx files to .ePubs with pagelists or pagemaps. However, the latest experimental Kindleunpack version should be able to reverse-engineer the original ePub file, including a pagemap, if the .mobi book hasn't already been processed with Calibre.
