It looks like your library's proxy is blocking cookies from EBSCOhost. I have made a patch to the plugin that should handle this.

It may take a few iterations to get this working since I am unable to test it myself. I am attaching a test version of the plugin to this post. You can download it and install to calibre using Preferences -> Plugins -> Load plugin from file.
