You might want to check the latest version of the Annotation plugin -- I think it is up to 1.42 which should work with either the QT5 or QT4 versions of Calibre.

You might have to manually download and install -- check the first message in the Annotations thread under Plugins https://www.mobileread.com/forums/sho...d.php?t=241206 where the latest version is maintained.
