The problem is most likely with the Kobo database. I would delete the koreader.png file using your kobo. Then poweroff your device and restart it.
After that, following the instructions carefully, put the koreader.png back in the root folder and properly eject your device and disconnect the USB cable. You should then see, on your Kobo, processing the new media. With that done you can connect through the USB cable again and put the KoboRoot.tgz file into the .kobo directory. After safely ejecting the device and disconnecting the USB cable, you should see it updating.

The icon should start Koreader again.
