I seen the refusal to acknowledge a disconnect a couple of time. What I found was that connecting the ereader, waiting a minute and then disconnecting it again seems to -- finally! -- trigger the disconnect event. So far, that hasn't happened often enough for me to panic and a pinhole reset worked the one time that connect/disconnect did not work.

In regards to the suggestion that you might have a database issue, I'd recommend Calibre with davidfor's Kobo Utilities so you can easily run a database check along with backup and restore capability You can also use a SQLite database utility to do the database check but for most people, davidfor's solution is handier.
