Reinstalling Kobo Start Menu cannot help in this case. From your description I take it that the Kobo Start Menu tries to launch koreader, but koreader fails to run. Did you look whether there is an error log file in the koreader folder?
Another question: You said that you did two complete reinstalls and that you tried it again. Did you each time remove the old koreader folder (if there was any)? I am asking because in the past I saw it happen several times that koreader cannot handle one of the files it had created by itself (something like history lists, I cannot remember exactly) in one of its subfolders and this prevented koreader from running. Therefore, having such an automatically created file would result in what you described.
