See mrmikel's reply. Your bald assertion that you have a failing page doesn't support any argument. You could have put ANYTHING on those pages, some portion of it being corrupted or defective. What is it you want us to consider? One sentence without any code?
As I said: eBooks have structure; they are not simply conveyances for displaying whatever you want, wherever you want it. I have already told you how to create the effect you want. I'm sorry it doesn't suit you (not sure why
that is, either), but at this point, it seems that you are "square peg, round hole"-ing this. If you want actual help, post your "ePUB" here with everything, and let the good folks here take a look at it. Nobody can help you with this:

How come when I created three xhtml pages in Calibre -- placing the words test on the first and the third and an image in the middle -- only the first page would load up in all of the applications?

...as a description. It's also obvious, based on what you've posted so far, that you have no real background in ePUBs, and are expecting Calibre to do the work (and research/knowledge) for you. The fact that you say you created a text page, followed by an image that you designated "cover," etc., and
only the first page shows indicates some type of corruption in the document. No one here can guess what that is.
Post your test ePUB, and more than likely, you'll receive an answer. Or don't, and don't. {shrug}. The fact that this doesn't work the way you
WANT it to, or
THINK it should, doesn't mean it isn't working.
