That is why the calibre editor doesn't even attempt these sorts of things. There is not absolute answer to the problem that doesn't introduce another problem.
When you enter text directly in the book view, you are asking it to guess what you want. It is doing its best, but it isn't pretty. But any similar program will have the same difficulties.
Editing is best done in a word processor which then is published to an epub, using Toxaris' Word add in or AWP or Jutoh. They place the conversion at the end of all your editing, rather than the ongoing conversion of Sigil.
