I suggest that you look at calibre's built-in Modified(last_modified) column. That one
is updated when you update a book in anyway, even with 'update metadata' unchecked. But I don't believe it's shown by default.
Other than that, I submit that the Date is metadata and you told FFDL not to update metadata.

On the FFDL Config dialog, in the Basic tab, hover your mouse over the option 'Force Author into Author Sort?' and the tooltip will explain:
"If checked, the author(s) as given will be used for the Author Sort, too.
If not checked, calibre will apply it's built in algorithm which makes 'Bob Smith' sort as 'Smith, Bob', etc."
Calibre quietly uses 'Author Sort' lots of places that you might otherwise expect it to use 'Author'. Such as the author field when sending books to most devices. Try it, I bet that does what you want.
As for point 3, BookJunkieLI explained it perfectly well.
(The only custom columns I personally populate from FFDL are Updated date, Words and Chapters.)
