Dunno, because I wouldn't think of updating a program that's already running, so my advise is Yes it does matter.

Given its size I would suggest backing it up with one's libraries

My config is ~30MB. I put a symlink to my config folder in my CalibreData folder. My backup is configured to drill down symlinks; backing up changes to calibre config adds a few seconds to a daily 10 minute backup of all changes to my main data drive.

IIRC calibre stores MRU lists, window sizes, last used library and such in the config data.
