Nope it means where the heroine literally gets her bodice ripped and most of the time against her will. Most Bodice rippers don't even have a cover like that. Those covers didn't really become popular till the very late 80s and was the norm all throughout the 90s after bodice rippers fell out of fashion. Those are called heaving bosoms.
