Yesterday, I tried to change a cover image of a book I have in the Calibre bibliotheque, following drjenkins' advice. I succeeded so far, but after saving the replacing of the cover image and turning back to the bibliotheque view, the old image was still present. When I clicked on the book in order to open the book viewer, the new cover was shown. I achieved no changes by using the update metadata command, neither in the edit metadata mode nor by using the modify epub plugin. So, I opened the book in Sigil and saved it elsewhere; then I deleted the book in Calibre and fetched it again from the new saving place. Now there was a correct result.

Is this a bug in Calibre?
