If the conversion doesn't seem to work, double check that you've converted from your EPUB because the original format (in my case, PDF), does not contain the new manually entered TOC, only the EPUB does!

IMHO AZW3 is the better format for modern K's (but I am not sure if A' allows those to be emailed. )I strictly use USB
