If so, Lulu seems to be able to read all kinds of crap input when it comes to Pseudo-"HTML". Usually, all of TOC and images should be handled by automated processing from a quality source file. Since you do not have a quality source, you have to add those things manually, I guess. However, a native EPUB TOC would be impossible if you only upload an HTML. If you want to create one by using HTML elements, you might manipulate the HTML directly, and if you don't know how, a HTML editor might help. What Lulu does with an image reference on upload, depends on their implementation. Best chances are, if it is a URL, the uploader might download it from there and pack it into whatever file Lulu creates from the HTML, but there are no guarantees. Another option would be to insert the image afterwards into the EPUB and re-upload it.
