To your point 2, the latest builds have even an option to start Koreader with the last opened document.

Btw, with some of these crashes described in my last post this "last opened document" info got lost, although the last opened document was still visible in the history. I wonder why koreader does not start the last file from that list if on some reasons the info got lost which file was last...
